This form will serve 2 formats:

1- Individual Questions: No question is unimportant, but if we don't have a way to respond to your question we will never have the chance to address it where it matters. Please leave your info so we can get back to you.

2- Group Setting: When the Q&A is being used in a live setting where submissions & responses will take place in a group. This is the only environment where an anonymous question can be addressed.
